The Housing Stabilization Services (HSS)

The Housing Stabilization Services (HSS) program in Minnesota is a Medicaid benefit under the Minnesota Department of Human Services designed to help seniors and people with disabilities—including those with mental illness or substance use disorder—secure and maintain housing.

It includes assistance like housing consultation, helping people transition into housing (including covering moving‐expenses and deposits), and services to sustain housing stability once someone is settled.

However, as of August 1, 2025, the DHS announced plans to terminate the HSS program.

Provisions are being made for public comment, and DHS is coordinating with federal partners to minimize the impact on participants while adjustments and oversight reforms are developed.

The Thrive Together Model (Metro Program)

Serving adults across the Twin Cities metro, the Thrive Together Model offers personalized housing stabilization services that prioritize long-term independence and meaningful community connection. Each plan is tailored to the individual, not a standard checklist, ensuring participants receive the guidance, skills, and advocacy needed to transition from survival to lasting stability.

Housing Support – Minnesota Housing Support

Minnesota Housing Support (formerly Group Residential Housing) helps low-income adults afford safe, stable housing and cover basic living expenses. The program offers monthly financial assistance to make housing more manageable and secure.

Eligibility requires meeting income and asset limits, along with having a qualifying disability or being age 65 or older, and residing in Anoka County.
